Patient Comments and Complaints
Trent Hills Family Health Team (THFHT) strives to offer excellent care to all our patients. To reach that goal, we need feedback from our patients and/or from their families. There are several ways to communicate comments and concerns to THFHT.
A locked Suggestion Box is available in the Patient Waiting Room, at our main site in Campbellford. Please use the provided form to advise us of any concerns, suggestions or compliments. Forms and envelopes are available at all our clinics.

Feedback forms will be reviewed weekly by the Executive Director and acted upon accordingly.
Patient Satisfaction Survey
As part of our Quality Improvement Plan (QIP), which was submitted to the Ministry of Health and Long Term Care in March 2013, we will undertake a Patient Satisfaction Survey. The questions focus on booking appointments, length of appointments and patient involvement in health care decisions.
We are setting up this survey so that the data collected gives us the most usable information possible.
Also as part of our QIP, throughout the year, we will be tracking how many urgent appointments we are able to fill each day. This information will be used toimprove patient access to urgent appointments.
Patient Complaint Process
If you have a serious concern about the quality of care or any service provided by THFHT, please let us know. We will address your concern in a timely and respectful manner as per our Patient Complaint Process.
